A skilled nursing facility (SNF) is a residential facility that provides 24-hour medical care on site and activities of daily living (ADL). This may also be known as a nursing home and serves as a licensed healthcare residence for individuals who require a higher level of medical care than can be provided in an assisted living facility.
Our Medicare/Medicaid Certified nursing facilities provide skilled therapy and nursing services for both sub-acute and long-term needs of the seniors in our communities. We have seven skilled nursing facilities, including:
While the number of beds at each facility varies, each one provides multiple private and semi-private suites that are designed to feel like home. The rooms include a table/work area, flat screen TV, and telephone. Several facilities also include Alzheimer’s units for residents that may require a more secure and individualized level of care due to their special circumstances.
Some facilities stand out with unique features, such as the Sensory Room at Cullman Health & Rehab Center. This room provides stimulation and relaxation for all six senses. Benefits include reduction of tension, anxiety, agitation, and stress; and improvements to self-esteem, environmental awareness, communication, and attention span.
At the Folsom Center for Rehab and Healthcare, all three floors of the facility have an activity room and common area where fun events take place, along with daily socializing. Outdoors, this facility features a garden retreat that provides a beautiful place for relaxation for residents and families that are visiting.

USA Healthcare has one assisted living facility, which is Woodland Haus Assisting Living. This is a different type of facility than those listed above, because 24-hour skilled nursing services are not provided. We do provide senior adults with personalized healthcare in a comfortable, residential environment. This includes medication management and assistance with everyday activities, such as dressing or grooming, as well as housekeeping, food, laundry, and transportation services. At Woodland Haus, we always have staff members available to help with residents’ health, safety, and support.
Woodland Haus Assisted Living is a place where senior adults can enjoy true independence while still having the comfort and security of knowing immediate assistance is always close by.
Delicious meals are prepared and served under the direction of a Registered Dietician with special attention to “home-style” cooking. Residents select their meal choices from a daily menu. There are daily scheduled activities and functions as well as an exercise and wellness program. Rooms are fully furnished and spacious enough for personal treasures. Private or semi-private accommodations are available.

We want you to feel that your loved one is safe and secure, and we make sure that they are. We have many handrails installed throughout the facility, as well as a modern electronic “call for assistance” system in each room and bath area. Licensed nurses and certified nurse aides are on campus 24 hours every day, and there is always a physician on call.
Woodland Haus meets or exceeds all federal, state, and local rules and regulations for safety and fire resistance such as an automated fire sprinkler system throughout the facility, total fire resistive construction, and smoke/fire detection system wired directly to the fire department.
